At Power Chiropractic, we strive to streamline your visit to reduce stress and save you time. We are happy to verify your insurance coverage for you prior to your visit so you know exactly what to expect. We’ll also send you a link so that you may complete your new patient paperwork online before coming in—if that’s not convenient for you, simply arrive about 15 minutes early, and you can fill in the forms here in our office.
We suggest wearing comfortable clothing that you can move around in.
NEW PATIENT FORMS
Your First Visit
When you arrive, our friendly staff will welcome you and help you feel right at home. Then you’ll sit down with your chiropractor to discuss your health history, what’s currently bothering you, and your goals for care. We’ll conduct a thorough examination, typically including X-rays. We’ll explain everything along the way, and we’ll answer all of your questions.
Depending on your specific situation, we may provide chiropractic adjustments or other therapies at this visit.
Please allow one hour for your initial appointment.
Your Second Visit
At this visit, your chiropractor will go over the results of your exams and X-rays in detail, showing you the underlying cause of your pain or other symptoms and laying out our recommendations for care going forward. We’ll also explain all the costs of care, how much insurance will cover, and your financial responsibility.
If you’re ready to begin, we’ll get started with your individualized care plan at this visit.
This appointment typically takes about 30 minutes.
